The way a Conflict Sheet looks will depend on the cast, but the first thing I always do is have them fill out either a GoogleForm or something like WhenIsGood. Then, I plug their conflicts into a table, like below, and find all the free spaces. I like to have their names on the left column and the dates along the top, then I write int he times of their conflicts like in the example.
